Position Description: Reporting to the Executive Director, the RD will:

  • Assess patient nutritional status by gaining an understanding of food habits or preferences (socio-economic, psychosocial and cultural).
  • Medical background and clinical/biochemical profile.
  • Assist patients in making healthy food choices by developing nutritional care plans incorporating all the above factors and giving patients appropriate instruction and tools to self-manage their nutritional concerns.
  • Maintain complete and accurate patient records.
  • Deliver group education as needed.
  • Act as a resource person to CCFHT staff.

Promote awareness of Cottage Country Family Health Team (CCFHT) programs and services.

  • Continuously evaluate quality and cost effectiveness of nutritional care services and revising services as needed.
  • Network with local and/or regional RDs.
  • Maintain professional competence through participation in professional development activities.
  • Contribute to the mission, vision, and values of CCFHT.
  • Participate in CCFHT committee(s), i.e. Joint Health & Safety, Quality Improvement, etc.


  • Completion of a Dietitians of Canada Accredited Undergraduate Program in Nutrition or Food Science, and supervised Dietitians of Canada Accredited Internship or Practicum Program.
  • Three to five years of strong clinical experience.
  • Ability to practice independently.
  • Demonstrated ability to work effectively in an interdisciplinary team environment.
  • Experience in program development, implementation, monitoring and evaluation.
  • Status of “member in good standing” with the College of Dietitians of Ontario.
  • Ability to travel locally between sites as needed, providing own reliable transportation.

Compensation:  Annual salary between $70,000 – $74,000 based on experience, comprehensive benefits package, and participation in Healthcare of Ontario Pension Plan (HOOPP).
