NURSE PRACTITIONER, FULL TIME positions
Primary Location: Positions in Gravenhurst, Wahta and Port Carling
Cottage Country Family Health Team (CCFHT) is growing. We are looking for Nurse Practitioners (NP) to join our team working to their full scope of practice. CCFHT is committed to the Provincial mandate to ensure attachment for all people within our community, through Ontario Healths Primary Care for All initiative.
Position: Nurse Practitioner (Extended Class)
Full time permanent with benefits and HOOPP.
Parttime positons will be considered.
CCFHT is a collaborative, team based care environment. New Nurse Practitioners (NP) will join a supportive environment of existing NPs staff as an integral part of an interdisciplinary health care team. The NPs will be part of a larger team of primary care practitioners and allied health care practitioners providing access to primary care for a diverse community of patients. The NP practices are focused on attachment and as the MRP for a group of patients.
Roles and Responsibilty including, but not limited to:
- Performs the responsibilities of the position consistent with the operational policies of the CCFHT.
- Provides direct comprehensive care to patients with specialized and complex needs.
- Completes health assessments including health history and physical examinations.
- Works within the legislated scope of practice of the Nurse Practitioner, determines the need for, orders and interprets reports of appropriate laboratory and diagnostic imaging.
- Monitors the ongoing therapy of clients with chronic stable illnesses by providing pharmacological and counseling interventions.
- Determines the need for, and the implementation of health promotion and prevention strategies for families and individuals.
- Uses nursing strategies based on evidence-based information.
- Formulates and communicate medical diagnosis or disorder.
- Consults with a physician in accordance with the standards for physician consultation.
- Consults with and refers to other health care professionals and community services as needed.
- Gathers and documents statistical information as required.
- Participates in management meetings, professional practice meetings and appropriate task forces.
- Documents all encounters on an electronic medical records system ensuring that data quality standards for charting are met.
Qualifications:
- Current registration with and a member in good standing of the College of Nurses of Ontario in the extended class without restriction.
- NP work experience preferred.
- Current membership in the Nurse Practitioners Association of Ontario or registration with Registered Nurses Association of Ontario as an NP-EC.
- Primary health care experience, preferably in a community setting.
- Advanced proficiency in the use of computers and various software applications including Microsoft Office and electronic medical records systems (experience with Telus PS Suite is an asset).
- Demonstrated excellent problem-solving, organizational, and interpersonal skills with an ability to work effectively and respectfully in a collaborative, interdisciplinary team environment.
- Experience working with individuals with complex needs.
- Strong interpersonal, communication and organizational skills.
- Politically and culturally sensitive.
- High level of critical and logical thinking, analysis and/or reasoning to identify underlying principles, reasons, or facts.
- Experience working in a community setting.
- Ability to work flexible hours.
- Valid driver’s licence and reliable vehicle.
Compensation: Annual salary $125,000.00 to 128,000.00 a year based on experience, participation in Healthcare of Ontario Pension Plan (HOOPP) as well as an extensive benefit plan.
