REGISTERED NURSE (RN) – FULL TIME (1yr contract)
Primary Location: Bracebridge
Position Description:
Reporting to the Clinical Manager:
· assist in the development, implementation, and delivery of programming for older adult and senior patients;
· provide evidence-based disease prevention and/or chronic disease management educational and/or rehabilitation programs;
· conduct patient assessment and education appointments;
· provide group and one-on-one education sessions;
· promote awareness of Cottage Country Family Health Team (CCFHT) programs and services;
· attend continuing education and training relevant to CCFHT programs;
· contribute to the mission, vision, and values of CCFHT;
· participate in CCFHT committee(s), i.e. Joint Health & Safety, Quality Improvement, etc.;
· encourage patients to take action for their own health and wellness.
Qualifications:
· Geriatric Nursing Experience Preferred and experience with Cognitive Assessment is an asset.
· Registered Nurse member in good standing with the College of Nurses of Ontario;
· interest and ability to work with an interdisciplinary team;
· experience in disease prevention and chronic disease management;
· experience in provision of health education to patients;
· ability to travel locally and provide own reliable transportation;
· strong interpersonal, communication, and organizational skills;
· experience providing care to seniors an asset;
· previous education/certification in MoCA Cognition, MINT Memory Clinic an asset;
· experience in program development an asset.
Compensation: annual salary $76,150 participation in Healthcare of Ontario Pension Plan (HOOPP).

NURSE PRACTITIONER, FULL TIME position
Primary Location: Port Carling Health Hub
Position Description: The primary care Nurse Practitioner (NP) will practice to their full scope as outlined in the Standards of Practice by the College of Nurses of Ontario.
Including, but not limited to:
- Performs the responsibilities of the position consistent with the operational policies of the CCFHT.
- Provides direct comprehensive care to patients with specialized and complex needs.
- Completes health assessments including health history and physical examinations.
- Works within the legislated scope of practice of the Nurse Practitioner, determines the need for, orders and interprets reports of appropriate laboratory and diagnostic imaging.
- Monitors the ongoing therapy of clients with chronic stable illnesses by providing pharmacological and counseling interventions.
- Determines the need for, and the implementation of health promotion and prevention strategies for families and individuals.
- Uses nursing strategies based on evidence-based information.
- Formulates and communicate medical diagnosis or disorder.
- Consults with a physician in accordance with the standards for physician consultation.
- Consults with and refers to other health care professionals and community services as needed.
- Gathers and documents statistical information as required.
- Participates in management meetings, professional practice meetings and appropriate task forces.
- Documents all encounters on an electronic medical records system ensuring that data quality standards for charting are met.
- Contributes to the mission, vision, and values of CCFHT.
Qualifications:
- Current registration with and a member in good standing of the College of Nurses of Ontario in the extended class without restriction.
- 3-5 years’ NP work experience preferred.
- Current membership in the Nurse Practitioners Association of Ontario.
- Primary health care experience, preferably in a community setting.
- Advanced proficiency in the use of computers and various software applications including Microsoft Office and electronic medical records systems (experience with Telus PS Suite is an asset).
- Demonstrated excellent problem-solving, organizational, and interpersonal skills with an ability to work effectively and respectfully in a collaborative, interdisciplinary team environment.
- Experience and willingness to provide care of geriatric populations.
- Experience working with individuals with complex needs.
- Strong interpersonal, communication and organizational skills.
- Politically and culturally sensitive.
- High level of critical and logical thinking, analysis and/or reasoning to identify underlying principles, reasons, or facts.
- Experience working in a community setting.
- Valid driver’s licence and reliable vehicle.
Compensation: Annual salary up to $125,477.00 based on experience, participation in Healthcare of Ontario Pension Plan (HOOPP) as well as an extensive benefit plan.
