Cottage Country Family Health Team (CCFHT) is seeking a highly organized, adaptable and collaborative Primary Care Transformation Specialist to support the implementation of priority transformation initiatives across our organization.

This is a part-time, short-term contract position with a strong focus on project management, change management, digital transformation and hands-on implementation support. The successful candidate will work closely with CCFHT leadership, clinical and administrative teams, primary care providers, and other partners to help translate transformation priorities into practical, sustainable changes in day-to-day operations.

This role is well suited to someone who is comfortable moving between planning and implementation — organizing projects, improving workflows, supporting teams through change, helping implement digital tools, and working directly alongside staff when additional support is needed.

About CCFHT

Cottage Country Family Health Team is an interdisciplinary primary care organization serving communities across Muskoka and surrounding areas. CCFHT is undergoing a period of growth and transformation focused on strengthening access to primary care, improving integration across interdisciplinary teams, modernizing workflows and enabling new models of care.

Our approach to transformation is collaborative and practical. We want to build systems and processes that work for the people delivering and receiving care, while supporting staff and providers through the changes required to get there.

The Opportunity

Reporting to the Executive Director, the Primary Care Transformation Specialist will provide dedicated project and implementation capacity for a defined portfolio of primary care transformation initiatives.

The role will help bring structure, coordination and momentum to projects while ensuring that changes are implemented in a way that is practical for frontline teams.

Key areas of focus will include:

  • Project Management & Implementation
  • Coordinate and support primary care transformation projects from planning through implementation.
  • Develop project plans, timelines, workplans, action trackers and implementation tools.
  • Coordinate activities across clinical, administrative, operational and digital workstreams.
  • Track project milestones, risks, dependencies, decisions and outstanding actions.
  • Support meeting preparation, documentation and follow-up to ensure projects continue to move forward.
  • Translate broad transformation priorities into clear, manageable implementation steps.
  • Support testing, implementation and refinement of new workflows and processes.

Change Management

  • Support staff and providers as new workflows, processes, roles and models of care are introduced.
  • Help identify the practical impacts of changes on different teams and functions.
  • Develop implementation resources, communications, guides and training materials.
  • Facilitate or support staff engagement, workflow discussions and implementation sessions.
  • Gather frontline feedback and help incorporate it into ongoing improvements.
  • Help identify barriers to adoption and work collaboratively with teams to develop practical solutions.
  • Support change in a way that recognizes the importance of communication, engagement and staff experience.

Digital Transformation

  • Support the implementation and optimization of digital tools that enable primary care delivery and team-based care.
  • Work with clinical, administrative and digital partners to map and improve technology-enabled workflows.
  • Support adoption of new digital processes, tools and functionality.
  • Assist with workflow testing, implementation planning, training and troubleshooting.
  • Help ensure digital solutions support clinical and operational needs rather than adding unnecessary complexity.
  • Identify opportunities to reduce duplication, streamline processes and improve the flow of information across teams.

At-the-Elbow Transformation Support

A key component of this position is providing hands-on implementation support to teams during periods of change.

This may include:

  • Working directly with staff to implement new workflows.
  • Helping troubleshoot workflow challenges in real time.
  • Supporting teams during the launch of new processes or digital tools.
  • Providing practical coaching and support as staff become comfortable with new ways of working.
  • Developing quick-reference tools, templates and process documentation.
  • Identifying emerging issues and escalating or coordinating solutions as appropriate.
  • The successful candidate should be comfortable moving beyond recommendations and working alongside teams to help make change happen.

Qualifications & Experience

The successful candidate will ideally bring a combination of project management, change management, healthcare operations and digital transformation experience.

Qualifications may include:

  • Post-secondary education in healthcare administration, business, project management, health informatics, quality improvement, change management or a related field.
  • Demonstrated experience managing or coordinating complex projects involving multiple stakeholders.
  • Experience supporting organizational change, workflow redesign or implementation initiatives.
  • Experience working within healthcare, primary care or another complex service-delivery environment is strongly preferred.
  • Experience supporting implementation or optimization of digital health technologies, electronic medical records, workflow tools or other technology-enabled processes is an asset.
  • Formal training or certification in project management, change management, quality improvement or Lean methodology is an asset but not required.

Skills & Attributes

We are looking for someone who:

  • Is an exceptional organizer and can bring structure to complex or evolving projects.
  • Is comfortable working in an environment where not every answer is known at the outset.
  • Can translate strategy into practical implementation.
  • Has strong project coordination and follow-through skills.
  • Builds positive working relationships with clinicians, administrative teams and leadership.
  • Understands that successful change requires engagement and cannot simply be imposed through a project plan.
  • Can listen to frontline concerns while keeping projects moving forward.
  • Is comfortable learning and working with technology.
  • Can identify workflow problems and help teams develop practical solutions.
  • Communicates clearly and professionally.
  • Can work independently while remaining closely connected to the broader team.
  • Is willing to be hands-on and support teams directly during implementation.
