NURSE PRACTITIONER, FULL TIME position

Start Date- Jan. 2026

Primary LocationGravenhurst

 Position Description:  The primary care Nurse Practitioner (NP) will practice to their full scope as outlined in the Standards of Practice by the College of Nurses of Ontario.                                          

Including, but not limited to:

  • Performs the responsibilities of the position consistent with the operational policies of the CCFHT.
  • Provides direct comprehensive care to patients with specialized and complex needs.
  • Completes health assessments including health history and physical examinations.
  • Works within the legislated scope of practice of the Nurse Practitioner, determines the need for, orders and interprets reports of appropriate laboratory and diagnostic imaging.
  • Monitors the ongoing therapy of clients with chronic stable illnesses by providing pharmacological and counseling interventions.
  • Determines the need for, and the implementation of health promotion and prevention strategies for families and individuals.
  • Uses nursing strategies based on evidence-based information.
  • Formulates and communicate medical diagnosis or disorder.
  • Consults with a physician in accordance with the standards for physician consultation.
  • Consults with and refers to other health care professionals and community services as needed.
  • Gathers and documents statistical information as required.
  • Participates in management meetings, professional practice meetings and appropriate task forces.
  • Documents all encounters on an electronic medical records system ensuring that data quality standards for charting are met.
  • Contributes to the mission, vision, and values of CCFHT.

Qualifications:

  • Current registration with and a member in good standing of the College of Nurses of Ontario in the extended class without restriction.
  • 3-5 years’ NP work experience preferred.
  • Current membership in the Nurse Practitioners Association of Ontario. 
  • Primary health care experience, preferably in a community setting.
  • Advanced proficiency in the use of computers and various software applications including Microsoft Office and electronic medical records systems (experience with Telus PS Suite is an asset).
  • Demonstrated excellent problem-solving, organizational, and interpersonal skills with an ability to work effectively and respectfully in a collaborative, interdisciplinary team environment.
  • Experience and willingness to provide care of geriatric populations.
  • Experience working with individuals with complex needs.
  • Strong interpersonal, communication and organizational skills.
  • Politically and culturally sensitive.
  • High level of critical and logical thinking, analysis and/or reasoning to identify underlying principles, reasons, or facts.
  • Experience working in a community setting.
  • Valid driver’s licence and reliable vehicle.

Compensation:  Annual salary up to $125,477.00 based on experience, participation in Healthcare of Ontario Pension Plan (HOOPP) as well as an extensive benefit plan.

MEDICAL SECRETARY, FULL TIME

Start Date- Jan. 2026

Primary Location:  Gravenhurst

Position Description:  The MEDICAL SECRETARY plays a key role in supporting the day-to-day operations of a healthcare clinic. This position focuses on administrative and clerical responsibilities that ensure patients receive efficient, coordinated care and that clinicians are supported in their workflow.

Including, but not limited to:

  • Book appointments and distribute messages.
  • Maintain and update patient records in the Electronic Medical Record (EMR) system, including scanning, filing, and data entry.
  • Addresses patient inquires on the phone and in person.
  • Experience with EMR, particularly Telus Health PS Suite.  
  • Participates in preventative care management.
  • Managing outgoing referrals and diagnostic testing for patients and incoming results. 
  • Maintain office supplies and overall office functioning.
  • Participates on organizational committees. 
  • Ability to multitask in a fast-paced clinical environment.
  • Contributes to the mission, vision, and values of CCFHT.

Qualifications:

  • Graduate of a medical administration program and or related office administration course preferred.
  • Previous related clerical experience in a medical practice or hospital setting preferred.
  • Knowledge of medical terminology.
  • Excellent interpersonal, communication and organizational skills.
  • Exemplary customer service skills.
  • Ability to multi-task and set priorities while managing a high volume of phone calls and daily transactions.
  • Ability to troubleshoot basic IT issues. 
  • Experience working in a community setting.
  • Valid driver’s license and reliable vehicle.

REGISTERED NURSE (RN) – FULL TIME

Start Date- Jan. 2026

 Primary LocationGravenhurst

 The Registered Nurse (RN) will play a vital role in the delivery of cancer screening services, focusing on early detection, patient education, and health promotion. Working within an interdisciplinary team, the RN will provide direct patient care, coordinate screening programs, and advocate for equitable access to services across diverse populations.

 Position Description: Reporting to the Executive Director, the RN will:

  • Perform Pap tests, breast exams, colorectal screening coordination, and assist with diagnostic follow-ups.
  • Collect histories, monitor symptoms, and identify risk factors for cancer.
  • Teach patients and families about cancer prevention, screening schedules, and healthy lifestyle practices.
  • Work with physicians, oncologists, nurse practitioners, and allied health professionals to plan and evaluate care.
  • Maintain accurate records of screenings, interventions, and patient outcomes.
  • monitor patient progress and document findings accurately in patient records;
  • promote awareness of Cottage Country Family Health Team (CCFHT) programs and
  • attend continuing education and training relevant to CCFHT programs;
  • contribute to the mission, vision, and values of CCFHT;
  • ensure compliance with safety, infection control, and privacy regulations
  • participate in CCFHT committee(s), i.e. Joint Health & Safety, Quality Improvement, etc.;
  • encourage patients to take action for their own health and wellness.

Qualifications:

  • Registered Nurse member in good standing with the College of Nurses of Ontario;
  • interest and ability to work with an interdisciplinary team;
  • experience in cancer screening prefered;
  • ability to travel locally and provide own reliable transportation;
  • strong interpersonal, communication, and organizational skills;
  • experience in program development an asset.

REGISTERED PRACTICAL NURSE (RPN) – Full Time Temporary

Start Date- Jan. 2026

 Primary Locations:  Bracebridge, Archdekin  

 Position Description:  RPN will report to CCFHT Executive Director

The Registered Practical Nurse (RPN) provides high-quality, patient-centred care within their scope of practice, supporting physicians and the interdisciplinary team. The RPN delivers nursing assessments, treatment, health education, and clinical support to ensure safe and effective patient care.

  • Assess and connect patients from the Health Care Connect waitlist to available practitioners within Muskoka district.
  • Initiate nursing interventions and strategies based on the best available evidence to assist individuals, families, and groups.
  • Conduct patient assessment.
  • Maintain accurate, timely, and complete electronic medical records (EMR)
  • Collaborate with primary care providers, hospital discharge planners/patient flow navigators, other program managers.
  • Prepare and organize patient charts, referrals, requisitions, and follow-up instructions
  • Assist with identification of quality indicators and support data collection.
  • Supports a patient-centric, population based, quadruple aim approach to program implementation and management.

Qualifications:

  • Registered Practical Nurse (RPN), member in good standing with the College of Nurses of Ontario.
  • Interest and ability to work with an interdisciplinary team.
  • Experience in primary care, community health, or a clinical setting is an asset.
  • Strong assessment, communication, and organizational skills.
